We need a statutory declaration signed in front of an authorised witness (police officer). In the numbered paragraphs, state the vehicle make and model, VIN, the reading currently shown, the reading required, and why the reset is needed. Please email us before sending anything so we can quote. Once we reply, send the signed original with your cluster.
